Safety, Loss Prevention and RAM
The Mondsworth loss prevention specialists each have over 15 years of experience in the oil and gas business in regulators, operator’s projects and consultancy. Their expertise has been provided to most major operators in the UKCS in the evaluation of safety related issues whether regulatory, technical, operational or strategic. This expertise is increasingly being called for in support of major projects.
The knowledge and experience of our team engineers enables us to deliver authoritative advice, training and technical solutions. Demand for our services is increasing in the UKCS as the life extension of aging assets requires new approaches to issues of design life, compliance, de-manning and increasing needs of utilisation of existing infrastructure for new marginal fields.

Safety Management
Mondsworth can provide advice that will help a client ensure that major accidents do not occur. We can also coordinate Health, Safety & Enviroment (HSE) activities on behalf of a client.
Mondsworth offers experienced personnel that can help customers plan, execute and audit Health, Safety & Environment (HSE) activities in accordance with internal and/or regulatory requirements. With detailed knowledge of relevant acts and regulations we are well placed to help customers establish and audit internal HSE activities.

Risk Analysis
Mondsworth has extensive experience in the planning, execution and implementation of quantitative risk analysis (QRA) techniques as a risk management tool. We have been responsible for the completion of a large number of QRAs ranging in size from small semi-quantitative studies of very specific activities to fully integrated QRAs of multi-platform offshore complexes.
We have undertaken QRAs for all stages of a project from concept selection through design, construction, operations, maintenance and modifications, to decommissioning and removal.
Hazard analysis and QRA are now well established methods for aiding engineering and management decision-making.
A full risk analysis involves the estimation of frequency and consequences (i.e. risk) for a range of potentially hazardous scenarios. The methods and tools Mondsworth employ to carry out QRA include:
- Hazard identification (HAZID) and HAZOP
- Frequency evaluations of all major events
- Consequence evaluations
- Fire, smoke and gas simulations utilizing the CFD tool CFX
- Explosion simulations utilizing the CFD tool FLACS
- Impairment evaluations
- Quantification of risk to personnel, environment and asset
- ALARP evaluation
QRA does not necessarily have to be a complex exercise. In some cases a relatively simple QRA suffices. The ability to accurately predict to what level a QRA should be developed, at an early stage of a project, is key for the creation of a cost-effective and useful, decision-making tool during the life of the project.

HAZOP, HAZID & CRIOP
HAZOP, HAZID and CRIOP are effective methods for assessing technical systems as well as procedures.
HAZOP
HAZOP(HAZard & OPerability analysis) is an well established method for identifying potential safety and operational problems associated with the design, maintenance or operation of a system. A HAZOP is a formal and objective process, where a group assesses the different parts of a given system with the aid of “guidewords”. This ensures a systematic and well documented evaluation of potential problems/hazards.
HAZID
HAZID (HAZard IDentification) is a high-level, systematic assessment of a plant, system or operation intended to identify potential hazards. This method is often used as a basis for risk assessment.
CRIOP
CRIOP (Crisis Intervention in Offshore Production) is a structured method for assessing offshore control rooms. The main focus is to uncover potential weaknesses in accident/incident response. CRIOP assesses the interface between operators and technical systems within the control room. The assessment is comprised of two main parts: (1) a design assessment in the form of a checklist; and (2) a scenario based assessment intended to assess the adequacy of response to critical situations
